Health Coaching & Mentoring Client Agreement

In ticking that I accept the Health Creation Terms and Conditions I am agreeing that I understand:

As a Health Creation Coach and Mentor Silvia Grisendi provides the client with holistic support, guidance and motivation in making healthy lifestyle changes. Although she has other professional qualifications, in the Mentor role she is not working as a nutritional therapist or functional medicine practitioner. Her role in this context involves helping the client to set up a personalised self-help programme and healthy lifestyle.

As Health Creation Mentor Silvia Grisendi observes strict confidentiality, unless required otherwise by law. Client data and notes are held by her, and by Health Creation where the Online Health Creation platform is used alongside coaching sessions, in strict accordance with the GDPR privacy regulations. Silvia may discuss issues raised in a client session in her Supervision or Mentor Development sessions, but they will do so anonymously. Written work that the client may do within the Online Health Creation platform, or by the use of digital forms, will be visible to Silvia and Health Creation. This means that anything that the client wishes to keep private should not be written into these means, kept in a personal notebook.

​Health Creation Coaching and Mentorship Programme
Our work with clients and the results they obtain tend to be more effective if they make a commitment to work within a recommended time frame. However, we reserve the option to apply flexibility and adapt timelines based on each client's needs.

Duration: Ideally, the Programme (six monthly sessions) should be used within a maximum of 8 months, after which any remaining sessions will be foregone (and we will not be able to offer you a refund), unless we agree on a different schedule.

Payment: fees must be paid in advance (through the website), upon agreement of a start date to secure your place and confirm your first appointment. Bank transfers can be arranged, if preferred.

​Cancellations: sessions which have been booked can be moved by mutual agreement. However, sessions must be paid for where less than 24 hours’ notice is given, except in emergencies at the discretion of the Mentor.

Missed appointments: failing to attend a scheduled appointment without any warning will results in the appointment being forgone from your programme.

Refunds: If a client wishes to cancel the Mentoring service altogether, hours which have been paid for but not used can be refunded. If after the first paid hour the client is dissatisfied with the service, fees can be refunded,

Privacy and personal data
All information the clients provides will be kept private and confidential. Shanti Wellbeing will not contact members of your medical team unless we have discussed it with you first and you have given your consent. The ultimate responsibility for your health care is your own and we are here to support you in this. We reserve the right to discontinue our services where it is apparent that your expectations, and what we can provide, are not in agreement.

Standards of health coaching and mentoring practice are recognised through UKIHCA, through which Health Creation training and qualification is accredited and maintained to the highest standard.
